The DnD Heroes of Horror book had a table of "Creepy Effects." It's one of my favorite tables of all, and includes effects like:

-A PC's Holy Symbol begins to weep blood from nonexistent wounds.
-PC discovers pieces of the last monster or person they killed mixed in with their rations.
-Everyone in a town the PC's are passing through stops what they are doing and intently stare at the PC's. If questioned the entire town continues moving, working, having conversations etc. and act as if nothing ever happened.
-The sound of a whetstone sharpening a blade not far off can clearly be heard, yet neither the direction nor location can be discerned.
-A PC awakens hearing chewing noises inside their bed, with no sign of what had made them.
-After a few hours in a new town, it slowly dawns on the PC's that there are no children.

-A slain human opponent dies in a manner reminiscent of a monster--crumbling like a zombie, for instance, or letting out a certain horror's death-rattle as he falls.

- The character is asleep in his bed, and he wakes up to the sound of drops falling on water. He looks around and the bedroom floor has turned into a black ocean in which he can see things moving underneath the surface...black, tentacled things (and they're not octopi).

- When the PC fires a round from a gun, he is thrown back 10ft with a "noisy cricket" amount of recoil, and the gun spews out a massive, pressurized gout of blood.
Damage for the shot is tripled. Later analysis of the blood reveals that each individual cell is from a different species of creature.

-When walking down a hallway (mansion, hotel, whatever) the players suddenly notice a set of bloody footprints being imprinted on the carpet, passing from one wall to the other, as if an invisible person just walked through one wall and continued across the hallway through the other wall.

-Call for a will save from all players once in a while, and those who fail will begin to hear a low whispering chant that repeats a few times. "Sath Ar Kathool. Ie mthach Agh-Mnyorliathep." (This event works well if you use it frequently during a game.)

-The woods are completely silent. Utterly silent. And empty, for that matter, even of mosquitoes. As a counter: the woods are filled with chirps, and the sounds of thriving life. But every creature seen is dead, and in a fairly advanced state of death.

-When an enemy (should be a normal cultist, thug, etc.) misses a PC with a gun, roll damage as if it were a critical. Apply the damage and ask the player to make the massive damage save. The damage should be enough that the player fails. Describe in detail the bullet passing through the character's head. Then the process reverses. The PC can feel the flesh, tissue, and bone being put back together at high speed. The bullet flies out of the wound and leaps back into the gun.
Time restarts. This time, the gun jams and explodes in its owner's hand. The man is killed. Seconds later, the PC falls to the ground and begins to vomit. The mass contains, blood, shattered bone, and brain tissue. If this is run through a lab for testing, it is found to be an exact genetic match for the PC...

-The party turn the corner into a fairly long, nearly empty corridor, with occasional bits of furniture. As they walk down it they see spectral figures approaching from the other end. If they stop, the spectres stop as well. The closer they get to the centre, the more clear the figures become: they are the parties own reflection, in a sheet of glass, placed inoccuously, near invisibly at the centre of the corridor. when a player get close enough to examine this, the reflection glares at the player, grabs a nearby chair and hurls it through the glass, causing it to shatter completely.

• A group of screaming men, all wearing street gang clothing carrying knifes, guns, etc. Their legs and arms are all bent out of position and erupting from their faces are long, bloddy, bone-colored stalks that have remnants of their faces wrapped all around it. They make noises as though they were starting a fight or shouting obscenities, though they not speaking any human language.

For that matter, just call random Spot and Listen checks. The party isn't rolling against anything, but they'll wonder what they've missed.

-In an abandoned building, the lights turn on and off, as if someone is walking around the room, turning the lights on as they enter, and off as they leave. If the PCs wait until it approaches them, the lights will be on in their room, the adjacent room will turn off, there will be a pause of complete silence, before the light in that room turns off, leaving the players in darkness....

-For any, any reason, the PCs reach a seemingly silent and beautiful hill somewhere in the wilderness. They encounter the "village's fool", who claims that the hill ate his herd. Let them laugh and continue on their way, just as the hill opens his eyes and his gaping maw...

-The players befriend a shy, agoraphobic girl who says she can help them. She claims to be in love with an Angel, but being in an asylum its likely that she is just insane, that it is just a split personality or method of soothing her trauma. She escapes from the asylum after something particulary upsets her (a new uncaring counciler, the players doubts, etc). The players eventually find her in an abandoned, run-down house. A shadowy, tentacled being is holding and caressing her tenderly as she cries tears of relief.

-A vagrant claiming to be Jesus (or some other religious figure) returned to Earth is killed. All of the icons of that figure are seen weaping that evening.

-While the PCs are walking down a street at night, a power shortage takes all the lights out, leaving them in darkness. As their eyes become used to the dark they realise that every streetlamp on this street has a dead body hanging from it, and the streetlamps themselves look a lot like gallows, and that the stars and moon are nowhere in the sky anymore...

-When the PCs are in a facility, expecting resistance from guards, give it to them. When they're in a place with only one exit, which they came in from, and just outside the reactor or virus vault or whatever, have the last set of guards die violently or step aside to let them pass, anwering any questions or violence with a headshake. If the party is expecting resistance, a clear path for them for no reason will really throw them off.

-One of my favourites, just because it's useless and cruel: the characters find a small, smooth cube covered in incomprehensible runic writing and made of some black glass or onyx-like material that they can't identify (the bait). If a character examines it closely enough (maybe a Spot Hidden roll or whatever), he or she realizes that, inexplicably, it only has 5 sides. 1/1D6 sanity loss at this realization. Could be a major plot-artifact, or just a cruel joke.

-While exploring some dark area through use of torches, lamps or flashlights, all colors slowly and steadily greenshifts from a pale tinge of green to a very deep, malevolent green. After the players panic, an audible pop is heard and everything is back to normal.

-Upon cutting a fruit, you find meat inside.

The PCs are told to oversee the digging of a large trench by bulldozers on an isolated part of the base away from public view. Trucks drive up - the PCs help unload barrels of gasoline and drums of quicklime. Next to arrive are trucks containing heavy machine guns -The PCS have to set them up on tripods so they will sweep the edge of the trenches. A staff car turns up - A Five star General gets out along with two men in Black suits and sunglasses and makes a short speech - the substance of which is that sometimes a soldier has to obey orders without questioning them. The next vehicles that pull into sight are a line of buses, yellow school buses with shades drawn over the windows. The sound of the coach engines cannot quite mask the sound of children’s voices singing "There was a farmer had a dog and Bingo was his name-o"...

-A king offers the PCs dates. The dates are horribly addictive, and the king uses this to his advantage.

-The moon is alive.

- Strange writing on the walls (choose your medium) says "come home (PC's Name)", however, the PC can not see or sense the text, and must be informed by friends of the message.

Upon searching the library of a large house or mansion, the players discover that every single book in it is blank. If they begin flipping through the pages anyway, a word shows up here or there, in a way that makes them think they missed it before. If they continue flipping through the books they will encounter essays and stories about people that sound remarkably similar to them. If they keep investigating, the books start dropping their names, and eventually addressing them directly.

Players continually encounter what appears to be the same homeless man over and over. He need not interact with the players at all, but if questioned, he denies knowing anything about it. If he is killed by the players he will keep showing up but will have an odd-looking scar that corresponds with the killing blow. If the players have some means of forcing the truth out of him or using clairvoyance to discover what's going on, it is revealed that he has no idea what the hell they are talking about.

If your party is a Hack and slash kind try this

Things don't die when your PCs kill them, not in any supernatural extra challenging way but in that the skin is still warm, blood continuously trickles out, the eyes on the freshly decapitated foe follows you around the room, and no traces of necromancy is found. If your Party tries to bring any chunks such as a hand to a wizard or priest to figure out what is going on they will notice that the wounds are slowly closing. When they get to their destination and show the specimen it is comply rotten, never offer an explanation for this.

Players are invited to a dance club. All players are drawn to the dance floor except one.
As the strobe light kicks in, the last PC watches as the dancers randomly flash from regular people to long-decomposed versions of themselves.

This only happens briefly, then as the strobe light stops, they return to normal. Even when the strobe light turns on again, the others remain normal looking.

a there mirror in a local inn room. the some of the PCs stay in that room. if one looks at the mirror and stares long enough he or she will see the reflection move without him/her moving. any other PCs in the room will just see a normal reflection of everything.

I remember a thread here a while back about this, and downloaded a 90+ page word document compiling things like these from various sources. I recognize a lot of them from >>1791120, >>1791123, >>1791139, etc.

Two of my favorites from that particular list are:

-A cat trots out from behind a corner, and lays a heart at the character's feet. It goes back behind the corner, and lays a liver there. It does the same with a pair of lungs, some kidneys, a stomach and a brain before imploding with a slurping noise. Autopsy shows that the organs were the cat's.

-A puddle on the road has a drowning woman hammering on the bottom of the surface as if the water was solid.

-One of the pc's comes across a kid who looks remarkably like them when they were that age. Upon closer inspection, the area is very remenisent of where the pc grew up, and further, he recalls witnessing this event from the child's perspective..

-The pc's wander in to a village, but after a while they notice there are no elderly or anyone above the age of 30. The local taverns on the other hand are providing very cheap meat based meals.

-The statue of the local hero in the town square has blood stains running down from it's eyes one morning. Long time admirers of the statue also complain it's position has shifted a bit.

-During the night, a far away field shows thousands of campfires. In the morning, there are no signs of them, but local legend tells of a large battle happening around here.

-Upon slaying a skeleton in a dungeon, it screams and collapses on the ground, a now dead, fully fleshed commoner.

Go into an elaborate description of a room, but add a brief description of an NPC or Monster half way through your description. If your player(s) ask you about it, profess ignorance.

"The room is filled with dust, simply opening the door sent enough into the air to make a thick, if temporary, fog. Only a small amount of moonlight filters in from the room's only window, hindered slightly by the thin moth-eaten drapes. The scant lighting makes it difficult to make out the details of any of the room's paintings, or the face of the crying child in the corner, and in fact the small amount of light may account for the way the mold growing on the walls seems to shiver and crawl ever so slightly. Across the room is a door with a small bloody hand-print which seems to be beckoning you."
"Alright, I'm going to approach the child cautiously with my hand on the hilt of my sword, and ask her what's wrong."
"What little girl?"
"The... The little girl crying in the corner?"
"What the hell are you talking about?"

Only try this one once or twice in the course of a campaign, it's pretty risky and hard to pull off properly.

I just thought this up upon using my 'new' keyboard.
It's an old style, with mechanical keys so it's all 'dakka dakka dakka'.

PCs going around doing stuff, have them make listen checks, really hard at first, but then easier and easier until they don't even need em anymore.
The sound they hear is typing/writing.
Basicly it's an unseen force recording their actions verbatim.

The party comes across a town that quaries a neighboring dead town. The people from the old town were turned to stone, and the neighboring town uses the stone to build with. (perhaps if the curse that turned the town to stone is reversed all the stone taken will turn into blood and flesh).

At length he's finally caught and being interrogated, and manages to croak out the same question, why there was a corpse hanging in the center of town. The chief of police says, "To catch any we missed." And then his face opens up.

The story ends with another man spotting a fresh corpse on a telephone pole in his town.

Upon grabbing the dungeon treasure and heading out, suddenly the cats are gone, and in their place, horrible monsters casting shadows of cats around the corners.

-The pc's discover a chest in the woods. Upon opening it, they find a painting of the party with an unknown marks painted on their foreheads, each with a different mark.

-In a village, one morning a second well appears next to the main village well, an exact copy, but only black water comes up from it's bucket. Should anyone drink it, they see the villages (and only this village, not the pc's either) people replaced with monsters and demons.

In the morning have them realize that the clearing is a perfect circle with their campfire in the middle.

Those that succeed, inexplicably discover that they made camp no more than thirty feet from the front door of a handsome inn, firelight radiating from the windows. The PCs gets sick during a night's sleep..
Each one has a symptom:
Sore throat, Ear infection, Fuzzy vision, Bloody vomit, or Rash.
They start off very minor, something that could be brushed off as something else.
As the symptoms get worse, it turns out to be more than being sick.

Sore throat becomes crackily then mute, and when s/he tries to speak: the sound is like the cries of a abused child.
The Ear infection becomes hard of hearing then completely deaf, and finally begins to hear things that are not human.
The Fuzzy vision becomes blurry and then like TV static, not long after images appear in the static, images that should make the PC want to rip his eyes out.
The Bloody vomit just continues, but the PC loses all will to eat, despite being weak and on the brink of starvation s/he will never succumb to it.
The Rash starts as a bug bite, then poison ivy, then something along the lines of an STD, the skin will not blister or fester on its own, but the PC will need to make more and more throws to prevent himself from scratching his neck with his own long sword.

In one room of the dungeon, all the furniture in the room has been Animated and Awakened. The chairs greet the PCs when they enter, and tell them to make themselves comfortable. They offer themselves up as amenities, and willingly oblige any whims the PCs may come up with, as long as they are not violent in nature.

However, if the PCs ever try to leave the room, the objects begin to cry and whine, complaining of being lonesome. After a while, they devolve into curses, and scream and yell at the PCs, driving them out.

If the PCs try to enter the room again, they find the objects have piled themselves up, blocking the door until the PCs apologize. If the PCs refuse... they set themselves on fire, blaming the PCs for their suicides. The door catches fire very quickly, and the inferno becomes nearly impassible.

After an hour or so, the flames die down, and among the ashes are charred, blackened human skeletons...

If the PCs kill the furniture instead of putting up with their drama, they find that the furniture bleeds when hit.

-After a fun night in a tavern on the trail of their adventure, one of the PC's discovers a diary hidden under one of the beds. Each page describes the same series of events that the PC experienced that night in exact detail and in their handwriting, over and over again, each time described different (ie: not the same paragraph over and over) The last few pages have additional stating "I can't tell how many days it's been, we have to break free of the cycle before the sun rises or it'll start again!"

-It's a bright and sunny morning, but there is no sun in the sky. It shows back up the next day.

-One of the pc's left eye suddenly starts seeing things. monsters off the edge of their vision, hidden attackers, things moving in the distance coming for them, but only with the left eye. the right eye doesn't see any of this.

- One of the players finds a VERY plain white mask. Whether they take it or not, it keeps appearing in unexpected places, even if it's presumed destroyed.

- A sound like a thousand plates wobbling on a floor comes from the next room over, but there's nothing there, let alone any plates.

- When a PC is trying to sleep footsteps can be heard coming down the hall towards their room door. The door then opens and the footsteps continue to the side of the bed, but nothing can be seen.

The party spends the night in town. The next day, the town is attacked by a necromancer and a small army of undead.

Your party heads out to fight the oncoming threat, and when said character locks eyes with the necromancer, the 'mancer smiles and the rest of the party notices that the character's clothes are covered from eye level to toe with dried blood. But when he cries, the tears are clear.

Every time a PC wakes up, they find a flayed corpse at their feet. Nearby (written in blood on the wall, burnt into the ground or even carved into the chest or face of the corpse) is the message "I BROUGHT THIS FOR YOU".
This happens even if the PC is just knocked out during combat, and the other party members will see the corpse and writing simply appear out of nowhere. If the PC in question dreams, it will always be of vague terror and desperate feelings of insane love and they will awake with peals of mad laughter still ringing in their ears.

Have NPC's disappear from the sight of the PC's at first very rarely, and then frequently. (e.g. The Shopkeeper smiles and hands you your weapon, you reach into your pouch to pay him but as you look back up the Shopkeeper is gone. As you turn around you see your friends in the shop still browsing, the door closed. A hand taps you from behind and the Shopkeeper asks for his payment) Eventually have one of the important NPC's disappear completely and when the PC's question on the NPC's whereabouts everyone does not know who they are talking about.

Leave if for a few sessions and then start the PC's losing line of sight to each other.

If I might expand on the idea, the DM should insert pleas for help and the like into his normal descriptions, preferably without a tone to emphasize it. For example:

"You walk into the library. Everywhere around you, you see dusty old books, covered in dust and mold, the wear and tear of ages help me covering the undoubtedly valuable- yes, what? I'm not done yet."

"What was that you said?"

"What?"

"You said help me."

"No I did not."

More points if the rest of the players heard it too, and you convince them that either they're all going nuts, or you have. Even more points if you say this after the 'little girl' thing; make the players think you're possessed or something.

The PC gradually becomes aware that something is trying to help them, but it's ideas of 'help' differ greatly from their own.
A 'simple' (read: clearly as dumb as a brush) country yokel sitting on a fence jokingly pokes fun at the PCs strange attire, but when he gets to the 'chosen' PC he cracks a joke and then goes down in a spray of blood as he is horribly gutted by some unseen force.

The PC is moving through an area that is extremely cold and the party is forced to make camp for the night. The PC thinks he's in for a hard night but sleeps surprisingly easy, dreaming that he's being held in the warm embrace of a loved one. When he wakes up he feels a hand stroking his forehead and his face is wet and covered by a warm blanket; on removing the blanket he finds it is a human's skin. There is no sign of anybody for miles, but the PC senses a pleased presence nearby.

While walking along, the PC senses constantly that there is someone behind him. If he looks around, there is nobody there, no matter how fast he turns. However, if he stops suddenly or otherwise moves backwards, he bumps into something and falls over.
If he does neither of these things, something treads on the back of his shoe and he feels a burst of sympathy from somewhere.

Of course, all these are kind of concentrating on the one player. I realize this isn't about RPing, but the story about the shoe reminded me of this.

There was a pair of socks that I had and I liked. I remember this pair of socks because they had a little silhouette sewn in on the ankle. A d00d. I purchased this pair of socks for myself at a Target when I was driving to Colorado to visit some friends over Spring Break. With cash.

Nobody knew about the socks. Unfortunately, I tore the new sock at the heel on the second day of the trip. Threw the first one out when I got home. Did the laundry, and as expected, the other sock in that pair came up without its partner. Knowing its partner's fate, I threw it away in the kitchen trash and took the trash out. Then it shows up in my desk drawer. I throw it away again, this time in the bathroom trash. That trashcan is full as well, so I take it out. Sock shows up in my pocket a week later.

Frustrated, I cut the sock into pieces (roommates had moved away for the summer, nobody saw the ritual) and threw it away in the dumpster outside.

One week later, I get a letter in the mail without a return address and with no stamp. In it is the sock.

To this day I still have no idea what happened. And, yes, I still have the sock.

Upon awakening the PC's notice that something isn't right, but they can't immediately place what is wrong. A particularly observant PC might eventually realize that the entire world seems to have reversed its orientation, now appearing exactly as the world does when looking through a mirror. If the PC DOES look through a mirror they will see the world in its "normal" orientation as they are used to. Everything else however is completely normal, including the PC reflections looking back at them through the mirror with the same perplexed expressions on their faces.

The PCs arrive at a town modeled after a graveyard/tomb/etc.
As they approach the main gate a eerie bell and wails of the dead can be heard along the lines of
"The town is under attack"
Bum rushed by skeletons/ghouls they take refuge in a building where they find a larger 'adult' female ghoul blocking them from smaller 'child' ghouls.
They catch a glimpse of their reflection in a mirror or pane of glass and see an introverted reflection.
[They look undead and the town looks normal]

- The PCs enter a crypt thats incredibly dusty, however it doesn't matter where they are as long as its certain someone will sneeze.

Whenever any PC sneezes unwillingly make them blink, their vision becomes vivid images of dæmons or mutated beasts about to strike them. However it is only for that short duration when their eyes were closed during the sneeze, not when they regularly close their eyes.

One I've used in a horror campaign before.

Characters come across a hallway choked with thick cobwebbing(not sticky webbing). Instead of burning or cutting it away, they just pushed through it. I had the characters roll fortitude saves when they passed through, but then "nothing" happened to the characters.

The next morning I had them all roll fortitude saves again. Those that failed this save took 1d4 points of constitution damage and were sickened. Later on that day the characters that failed thier second save had to make a final save. The ones that failed this save were nauseated for one minute, afterwhich they each vomited up a swarm of spiders which promptly attacked the party.

In a snowy, mountainous pass, the players happen accross an abandoned campsite. The tent appears to have been violently torn open on one side. On close inspection, it was opened from the inside by numerous frantic slashes. Several hundred meters away, two fresh bodies are found beneath a tree, shoeless and dressed only in underwear. Another three bodies are found between the tree and the camp, dressed partly in strips of clothing cut from the other victims. Tracks in the snow make it clear that they were trying to return to the tent. If the players search carefully they may discover four more bodies that were deliberately buried in the snow: naked, toungeless, and with their eyes burned out. There are no tracks or any other indications snowing any outside involvement at all.

Variation on this: An ax hangs next to the mirror. When the PC comes too close to the mirror, he sees his reflection reach to the side of the mirror, pull out an ax, and swing at the glass. The glass shatters, falling to the ground at the PC's feet. Behind the mirror is a wooden wall, now with an ax head half protruding from the wood, coming from the other side. The ax to the side of the mirror is gone.

The innocuous should be frightening.

One morning your characters wake up to to find the world veiled in a coloured haze in hues of blue. This becomes more and more prounounced over time until the whole world is blue. Themselves, the sky, the clouds, everything is a differnet shade of blue. Except for one red rock in the middle of the road.

The rock never moves, never reacts to them, but no matter where they go, that same red rock is onscene.

The next morning, everything is back to normal. Thier very next random encounter, though(should be with something weak like goblins or kobolds) the first one they kill, instead of dying, shatters into dozens of identical red rocks. The other goblins scream in abject, soulcrushing terror at seeing this and flee screaming somethinga about "not letting in the colour".

One morning, when praying for spells, the paladin feels something... absent. If he's ever Fallen before, it feels a bit like that. When he opens his eyes, he sees in front of him a rolled up piece of cloth.
Rolled up in the cloth is a humanoid finger with a strange golden honey-like liquid oozing out of the wound instead of blood.

The current dungeon is the place where a mad scientist type has been performing horrible experiments.
The party finds a young boy, age 7-10 or so, on a stone slab, a blanket pulled up to his chin. He's sobbing and shivering. If they try to help, he rolls away from them on the slab, curling up and crying more.
If they take the blanket off him, they find a long line of stitches bisecting his torso, with a cross-line at his gut. He starts screaming about how <villain> put something in him, and why won't it come out?
After the party has had enough time to absorb this and start discussing what to do, the boy starts screaming "It's coming out, oh gods!"

when they go into the room, they find a sickly noble girl, scared out of her wits.

She tells them that the footsteps have followed her all her life, and noone else could hear them until now. She desparately asks them to get her out of there, to take her away.

When they try the door, its locked and apparently barricaded.

Next midnight, a pale man flickers into being, takes two steps towards the girl, and touches her arm. She dies instantly.

The door is unbarricaded and the PC's are freed, asked to never mention this to anyone.

The following night, the PC's hear the footsteps, far of. but, every night, coming one step closer...

The PC's are aiding a self proclaimed humanitarian gormet who's cooking has earned him accolades from all over the kindgom, who is trying to use his money and fame to help ease the hunger of the poor classes, and send the party to retrieve an item for him, supplying them with a map and supplies, including preserved meats.

As they work thier way down to the item, they contend with tribes of canabalistic humanoids of varying types. There should be plenty of descriptiosn of tearing the flesh from bones, blood and flensing tools laying around rust-stained sluices in the floor, etc. Eventually the recover the item, a 'leather' bound book deep in the temple that is penned in an unknown language.

When they return, however, the find that the chef has been executed for horrific cirmes against his fellow man... he was using "Humanitarian" in the same an elf might use "vegitarian"... which includes the food he sent the party out with.

The government attempts to confisciate the book and other items recovered from the temple. If the PC's do surrender the item, the next time they come by this town, they notice a deffinite drop in the homeless and starving populations, though no legitimate reason for it is ever provided....

- Upon staying in a new town for several hours, the PCs begin to notice that they always see the same townspeople outside, wandering, and they never see anyone entering or exiting a building. Further investigation will reveal that people inside rooms in buildings never leave the room that they're in, and no one ever eats or sleeps. If the townspeople are questioned, they deny anything is odd or strange. If the PCs try to force a townsperson through a doorway, the townsperson resists. If the PCs succeed in pushing the townsperson in or out, the townsperson vanishes, and no one in the villiage will acknowledge the dissapearance.

- On only one level of the dungeon, the PCs always have one extra shadow than the number of lightsources. PCs with darkvision or see all PCs having a single shadow. All other creatures and objects on that level of the dungeon have one less shadow than they should.

- If any of the PCs have a spiked chain, or shackles, fail, or any other chained weapon, the next time they use it in a fight, it collapses into a pile of metal links as the final enemy flees or falls. Inspection will reveal none of the links are broken.

- The corpse of a fallen enemy begins to twitch violently, and nothing anything the PCs do, even attempts to restrain it, can stop this motion. The corpse also seems rooted to a single spot on the floor and cannot be moved from that spot, as if it is stuck at that single spot with glue. Detection spells reveal no magic, no undead presence and that the creature is indeed dead.

Every time the PCs go to sleep inside a structure, whether they are in an interior room, or if the building opens to the outside, they awaken to find the door to their rooms hanging wide open. Tent flaps are found unfastened.

If a party member stays awake, or someone is on watch, it doesn't happen (seeing it happen invisibly would take away some of the creepy). But if they ever let down their guard, it starts happening again.

Have the stalker shred the poor boy to pieces clawing its way out of him, but seeing as it's completely undetectable (even with see invisible) the PCs can't really do anything about it.
Have it attach itself to a PC and follow them around, as if it were an animal following another animal it had mistaken for its mother. They go to kill the BBEG and it sees them hacking him to death and thinks 'Ah, thats what you're meant to do to people', perhaps.

- Have all the characters make spot/perception checks. Those that pass notice they can't remember the last time they shaved, cut their hair, or trimmed their fingernails, and none of those things have grown out at all since then.

- One of the PCs inexplicably find the first magical weapon or object they ever possessed. If they still possess this item, choose some other appropriately nostalgic lost piece of equipment that would be easily recognizable. The item should be found in a place where it should not be, such as a chest that has been locked for hundreds of years.

This stuff is bullshit, if you want to creep your players out, work it into the campaign/adventure somehow. D&D is full of these "tables of random stupid shit" that are stupid and shitty because they are random and pointless. If you want to have a holy symbol bleed, that's fine, but it has to be bleeding for a reason, otherwise your players will get sick of the dumb special effects and start to ignore them. The mark of a master GM is that he weaves all the strange little coincidences and eerie moments together into a single narrative, not that he rolls on a table and announces the result. That's not running a game, that's playing a halloween-themed slot machine.

there is an abandoned house on the outskirts of a town. it is the only place the PCs can afford to stay. on the way out of town the towns people say it is haunted, the PCs shrug this off and continue. they sleep soundly and joke about it. but when they leave the outside of the house is surrounded by dead animals. closer examination reveals that the bodied are forced in the shape of letters saying "Looks like we found some new friends to go with".

Several days into the adventure, have the PCs overhear rumors that the moon has stopped waxing or waning. Several days after that, have them notice that the moon has frozen in the sky, and is not moving, even during the day. (And it is still perfectly visible even with the brightness of the sun.) Several days after that, have the moon vanish completely. It returns at the start of the next month, waxing, waning, and moving across the sky normally, with no explanation.

Give the PCs a wallet of infinite coins that never runs out of gold pieces, but the money always has fresh blood on it. The magic doesn't create gold, it steals it from the nearest treasury, and in order for the magic to work it also must take the life of a bystander. Allow the PCs to figure this out moments before the authorities do.

Not EVERYTHING in the entire world has to be related to BBEG Death Knight Steve and his plan to take over the world using an army of zombie whores, and a littl flavoursome morsel like having your own undetectable murder-machine desperate for your approval helps add to character development.

The PCs come across a normal looking Old English Town. Rumors of an old castle far to the north harboring a fierce beast abound in a tavern which the PCs will undoubtedly visit. Somewhere along the lines a burly man will enter shouting about some girl saying no to his forced proposal. She seems to have also run off toward that castle. After he loses hope and he refusing a glass of beer, the entire town breaks out in song about how great that man is. the song talks about how good he is at expectorating as well as how he is a man among men. The song ends with him heading off to slay the beast with the townsfolk screaming 'Gaston!'.

It doesn't matter what the PCs do now.

When the PCs are preparing to bed down for the night in a small, isolated cottage, they are surprised by a loud clack sounding from the window. A ghoulish claw resting on the outside of the window is barely visible for a few seconds before withdrawing.

Searching outside reveals nothing. But if the PCs let their guard down at all, the claw returns for another few seconds, perhaps at at different window, or as a scratching sound across the door or the walls of the cottage.

If the villain of your campaign isn't interesting enough to be involved with these creepy effects, then your campaign sucks. It's like Batman Returns: tons of money for special effects and set design, not a dime for acting talent. So the movie blew.

What scares players is not surprise, is not confusion, it's suspense. When they hear footsteps in the mansion, if the players investigate it, they should be rewarded with something beyond a macabre scene that they have no control over. If someone leaves a note that says, "We are all inside" then they should do something once they're in there. Maybe it's drive the PCs out so they can't stay there again, maybe it's fight them with magic, maybe it's help them out and give them information, advice, or a gift.

The first rule of running games is: every random event is the opportunity for a meaningful event that got wasted.

That's why that works so well with the haunted ghost house. They weren't following the PCs until they stayed at the house, and they weren't inside the room until the PCs let them in. What else might the PCs accidentally do to make things worse?

Did you not read what I fucking posted? If the PCs go for that sidequest, they take down a minor threat unrelated to the BBEG, earn the gratitude of whoever sent them there in the first place, and get a fanatically loyal UNDETECTABLE KILLING MACHINE following one of the party members around.

It seems to me that having a BBEG in the first place doesn't mesh with some of the better ideas here. If a lot of the more subtle ideas in this thread ultimately turned out to be "lol it was a necromancer fucking with you" or "lol ghosts roll for initiative" it would completely kill the built-up suspense.

I think some of these ideas could be used for a campaign where something is increasingly "off" about the world as the players encounter more and more unexplainable happenings. Instead of hunting a BBEG, the players would have to investigate what is going wrong with the world/reality/the gods/the planes that is causing things to go haywire.

It doesn't have to be everything that happens everywhere in the world, but doing otherwise will put the players' perspectives way out of whack. If there's a sidequest, it should be something to further their long-term goals, either to defeat the villain or to accomplish personal objectives like establishing an order of knights. If you're going to have them complete sidequests just to get money and experience, or having invisible stalkers follow them around for no reason, there's no point in pretending your campaign is anything other than a pale copy of WoW.

It largely depends on whether you play low fantasy or high fantasy (or its approximate equivalents in other settings). If you're playing high fantasy with lots of epic conflicts between good and evil, you sort of need an over-arching enemy. OTOH, if it has more of a plucky adventurer feel, you don't really need that as much: players can go from smaller objective to smaller objective without having the doom of the world hanging over them.

And bizarre stuff doesn't have to be done by the evil overlord to be related; for example, in the "we are all inside" scenario, they could be invisible assassins that need to work in a team for a certain power to work, or they could be spirits that know something about how to defeat the ultimate enemy, or they could be the ghosts of people who he killed in that house many years ago, and can provide some more backstory.

Why is the woods full of dead animals and trees? Is it because the villain poisoned the land? Did he curse the forest? Is it an illusion? If it is an illusion, who cast it and why?

I guess what I should have said was that everything you add should prod the players into asking more questions, exploring things further, and eventually getting a better understanding of the situation. Just because it's a horror campaign, doesn't mean cause and effect don't apply.
